Alumni Transcript Request Form
This transcript request form is available only to Walsh Jesuit graduates and past students. Please complete the form below for each school / institution you wish to receive a transcript. You may request an unofficial or an official copy of your WJ transcript. What is the difference? A transcript becomes official when the school seal is affixed to it. Official copies can not be emailed; it is mailed via the U.S. Postal Service. *Please note: If an official transcript is opened by someone other than the college or organization it is addressed to, the transcript becomes unofficial.
